Forms
Resident students new to Mounds View Public Schools or entering from a nonpublic or home school setting must complete enrollment into the District. An enrollment packet with forms can be obtained from the school your student will attend.

Requirements
In addition to receiving this packet, you must provide a record of all student immunizations and proof of your residence within the school boundary area (driver's license, utility bill, rental/lease/purchase agreement). You may also be asked to provide a birth certificate. Contact the school for additional information, or to set up a time to register.

In-district transfers
Resident students of Mounds View Public Schools may apply to attend a school that is outside their school's attendance boundaries, yet still within the District. Applications for an intra-district transfer may be made based on the following reasons:

Intent to change residency - if the student's parent or guardian intends to move to a new residence within the school year and designates a mailing address for their new residence. Families must provide proof of intent to move, such as an accepted purchase, lease or rental agreement.

Daycare - if the student will be receiving daycare in the attendance area of a school other than the student's designated school.

Siblings - if the student's sibling already attends the school requested.

Employee request - an explanation must be provided.

A compelling educational reason - an explanation must be provided.

Like non-resident agreements, intra-district transfers are approved on a space-available basis, however, intra-district transfers are processed prior to non-resident agreements. Please note: 9th grade non-resident enrollment is closed for the 2008-09 school year.  

Applications are accepted year-round, however the priority consideration application deadline for 2008-2009 fall enrollment is February 1, 2008. To download an in-district application, click here. For more information or to receive an application packet, e-mail cindy.bruce@moundsviewschools.org. Application forms are also available in every school office and at Snail Lake Education Center, 350 Highway 96 West, Shoreview, MN 55126. Call 651-604-9782 for more information. Forms must be returned to the Student Information and Reporting Office at Snail Lake.
