  • NOTE: You must download ALL files to have the complete packet -- your application will not be accepted unless all applicable forms are included.
  • We are now accepting registrations by mail (you may still turn in your forms in person). To signup by mail, please mail all forms and include a self-addressed, stamped envelope for the return of your eligibility permit.
